- Please read all registration procedures and program information
carefully. Some programs have special instructions, deadlines,
and/or additional forms required.
- Complete the registration form.
- You are required to read the Program Waiver and Release form
and you must acknowledge the Release Form before your registration
will be accepted.

The Morton Grove Park District will accept payment for online registration
by Master Card, Visa or Discover only.
Additional Registration Information
All registration is processed on a first-come, first-serve basis.
Resident registration begins with the delivery of the brochure March
3. Nonresident fees for Park District programs are 30% more than
the resident fee unless otherwise stated. Nonresidents may register
beginning Monday, March 10. Registering on-line does not guarantee
you a place in a program. You will receive confirmation by mail.
Definition of Residency
Anyone living within the geographic boundaries of the Morton Grove
Park District is considered a resident. Students in K-8th grade
attending Morton Grove Schools (Hynes, Golf, Park View, Nelson,
Melzer, Edison, Jerusalem Lutheran, Julia Malloy and Muslim Community
Center) are considered residents for recreation programs only.
Refund Policy
Refund requests for recreation programs may be made on line, by
phone or in person. Refunds will be granted minus a $5 cancellation
fee for the following:
- If notice is given 2 full working days prior to the start of
a program.
- A prorated refund is issued after a program or activity has
started (medical, moving, etc.). Proof may be required.
- If a program is cancelled by the Park District a refund will
be made in FULL.
Morton Grove/M-NASR Inclusion Philosophy
The Morton Grove Park District and the Maine-Niles Association
of Special Recreation (M-NASR) believe that all individuals should
be provided with leisure opportunities that allow for performance
at their highest level of ability. Although many people achieve
this through participation in a M-NASR program, others may have
greater success in a Park District activity. M-NASR and the Morton
Grove Park District adhere to the the basic principles when providing
accommodations so as to allow for successful inclusion of individuals
with disabilities in Park District activities.
Reminders
Register early for programs! Some classes fill up very quickly.
If you wait too long to register, it might be too late. Unfortunately,
in some circumstances, we have no choice but to cancel a class if
the minimum number of participants have not enrolled prior to start
date.

I have read the Program Waiver and Release and
understand that my signature is required in order to participate
in Morton Grove Park District programs. I understand that this online
registration document shall substitute for and have the same legal
effect as the original form and that my typed signature will have
the same legal basis as an original signature.

Warning of Risk
Despite careful and proper preparation, instruction, medical advice,
conditioning and equipment, there is still a risk of serious injury
when participating in any recreational activity/program. Not all
hazards and dangers can be foreseen. Depending on the particular
activity, participants must understand that certain risks, dangers
and injuries due to inclement weather, slipping, falling, poor skill
level or conditioning, carelessness, horseplay, unsportsmanlike
conduct, premises defects, inadequate or defective equipment, inadequate
supervision, instruction or officiating and all other circumstances
inherent to indoor and outdoor recreational activities programs
exist. It must be recognized that it is impossible for the Morton
Grove Park District to guarantee absolute safety.
Waiver & Release of All Claims and Assumption of Risk
Please read this form carefully and be aware that in signing up
and participating in these programs, you will be expressly assuming
the risk and legal liability and waiving and releasing all claims
for injuries, damages or loss which you or your minor child/ward
sustain as a result of participating in any and all activities connected
with and associated with said programs/activities (including transportation
services/vehicle operation, when provided).
“I recognize and acknowledge that there are certain risks
of physical injury to participants in these programs/activities,
and I voluntarily agree to assume the full risk of any and all injuries,
damages or loss, regardless of severity, that my minor child/ward
or I may sustain as a result of said participation. I further agree
to waive and relinquish all claims I or my minor child/ward may
have (or accrue to me or my child/ward) as a result of participating
in these programs/activities against the Morton Grove Park District,
including its officials, agents, volunteers and employees (hereinafter
collectively referred as “Morton Grove Park District”.)”
“I do hereby fully release and forever discharge the Morton
Grove Park District from any and all claims for injuries, damages,
or loss that my minor child/ward or I may have or which may accrue
to me or my minor child/ward and arising out of, connected with,
or in any way associated with these programs/activities”.
I have read and fully understand the above important information,
warning of risk, assumption of risk and waiver and release of all
claims.
I accept the terms of the Waiver & Release of All Claims and
Assumption of Risk.

District policy if not acknowledged.
